Memorial Hermann Southeast Hospital recently opened a new Medication Therapy and Wellness Clinic to assist with the management of chronic diseases that require frequent medical intervention.
“Not surprisingly, we have found that supporting and being there for our patients produce the best outcomes; our patients feel better and their overall health improves,” said Pharmacy Director Rachel Samuel. “It’s a team effort – patient, clinical pharmacist, and physicians all working together for the patient’s benefit to improve his or her quality of life.”
The new Medication Therapy and Wellness Clinic focuses specifically on anticoagulation management, immunizations, pharmacotherapy review, and tobacco cessation. Clinic pharmacists facilitate care through the evaluation of patients and modification of therapy using medical staff-approved protocols.
